To apply for a refund of your IRS offset due to extreme hardship, follow this link to the Statement of Financial Status form. Complete the form and return it to:

The form must be COMPLETE and include all requested information along with a copy of the Department of the Treasury letter you received advising your Federal payment was being applied to the debt held by ECMC. Provide accurate phone numbers in the event we have follow-up questions.
